The distinction I'd venture to say has more to do with the number of guards and controls and restrictions than with the actual concentration of people.

It's ironic that many of the refugees of long standing will actually be refugees for the first time in their lives and those that weren't previously refugees get to see what it's like to be treated as refugees, after making sure that the early set of refugees maintain their status in camps.
